There are two ways you can complete a financial assessment form: either by printing the form or by filling it out online.

By completing the form online, you can securely send us supporting documents using the upload buttons provided within the form. This means you do not need to send sensitive information through the post.

How to complete the online financial assessment form

  1. Gather all your income, savings and expenditure information.
  2. If you have a printer, download and print the declaration form, which you will need to sign later.

    If you do not have a printer, please tell us and we will send you a declaration form to you.
  3. Follow this link to complete the online financial assessment form:

  4. Once you have submitted the online form, the information will be sent to us immediately and we will start the process of completing your assessment.
  5. Complete and sign the declaration form that you printed, or that we sent to you.
  6. Send the completed declaration form to us at the address below, or scan it and email it to us. Please also send us copies of any supporting documents that you did not upload via the online financial assessment form.

We will be unable to complete your assessment without a valid consent form.
