Team for People with Learning Disabilities

The Merton Team for People with Learning Disabilities and Transitions is a statutory agency which is part of the London Borough of Merton's Adult Social Care department. The team works with people with learning disabilities who are over 18 years old. This is a very wide ranging and diverse client group, which includes work supporting people who live independently in rented property, to commissioning and reviewing care home placements with high levels of support.

The integrated nature of the team means that practice takes place in the context of a wide variety of professional expertise, which social workers can draw upon. These include psychologists, behaviour support specialists, occupational therapists, a team of community nurses, speech and language therapists and team support officers. Outside of the team, we are in close proximity to many other colleagues in housing, welfare benefits, direct payments, direct provision services (services provided to people with LD by in-house council services) and shared lives (an adult placement scheme).
